BACKGROUND: Physical activity has many health benefits for young people. In 2018, WHO launched More Active People for a Healthier World, a new global action on physical activity, including new targets of a 15% relative reduction of global prevalence of insufficient physical activity by 2030 among adolescents and adults. We describe current prevalence and trends of insufficient physical activity among school-going adolescents aged 11-17 years by country, region, and globally. METHODS: We did a pooled analysis of cross-sectional survey data that were collected through random sampling with a sample size of at least 100 individuals, were representative of a national or defined subnational population, and reported prevalence of of insufficient physical activity by sex in adolescents. Prevalence had to be reported for at least three of the years of age within the 10-19-year age range. We estimated the prevalence of insufficient physical activity in school-going adolescents aged 11-17 years (combined and by sex) for individual countries, for four World Bank income groups, nine regions, and globally for the years 2001-16. To derive a standard definition of insufficient physical activity and to adjust for urban-only survey coverage, we used regression models. We estimated time trends using multilevel mixed-effects modelling. FINDINGS: We used data from 298 school-based surveys from 146 countries, territories, and areas including 1·6 million students aged 11-17 years. Globally, in 2016, 81·0% (95% uncertainty interval 77·8-87·7) of students aged 11-17 years were insufficiently physically active (77·6% [76·1-80·4] of boys and 84·7% [83·0-88·2] of girls). Although prevalence of insufficient physical activity significantly decreased between 2001 and 2016 for boys (from 80·1% [78·3-81·6] in 2001), there was no significant change for girls (from 85·1% [83·1-88·0] in 2001). There was no clear pattern according to country income group: insufficient activity prevalence in 2016 was 84·9% (82·6-88·2) in low-income countries, 79·3% (77·2-87·5) in lower-middle-income countries, 83·9% (79·5-89·2) in upper-middle-income countries, and 79·4% (74·0-86·2) in high-income countries. The region with the highest prevalence of insufficient activity in 2016 was high-income Asia Pacific for both boys (89·0%, 62·8-92·2) and girls (95·6%, 73·7-97·9). The regions with the lowest prevalence were high-income western countries for boys (72·1%, 71·1-73·6), and south Asia for girls (77·5%, 72·8-89·3). In 2016, 27 countries had a prevalence of insufficient activity of 90% or more for girls, whereas this was the case for two countries for boys. INTERPRETATION: The majority of adolescents do not meet current physical activity guidelines. Urgent scaling up of implementation of known effective policies and programmes is needed to increase activity in adolescents. Investment and leadership at all levels to intervene on the multiple causes and inequities that might perpetuate the low participation in physical activity and sex differences, as well as engagement of youth themselves, will be vital to strengthen the opportunities for physical activity in all communities. Such action will improve the health of this and future young generations and support achieving the 2030 Sustainable Development Goals. FUNDING: WHO.
The SOC-8 guidelines are intended to be flexible to meet the diverse health care needs of TGD people globally. While adaptable, they offer standards for promoting optimal health care and guidance for the treatment of people experiencing gender incongruence. As in all previous versions of the SOC, the criteria set forth in this document for gender-affirming medical interventions are clinical guidelines; individual health care professionals and programs may modify these in consultation with the TGD person.

BACKGROUND: In recent decades, the prevalence of obesity in children has increased dramatically. This worldwide epidemic has important consequences, including psychiatric, psychological and psychosocial disorders in childhood and increased risk of developing non-communicable diseases (NCDs) later in life. Treatment of obesity is difficult and children with excess weight are likely to become adults with obesity. These trends have led member states of the World Health Organization (WHO) to endorse a target of no increase in obesity in childhood by 2025. MAIN BODY: Estimates of overweight in children aged under 5 years are available jointly from the United Nations Children's Fund (UNICEF), WHO and the World Bank. The Institute for Health Metrics and Evaluation (IHME) has published country-level estimates of obesity in children aged 2-4 years. For children aged 5-19 years, obesity estimates are available from the NCD Risk Factor Collaboration. The global prevalence of overweight in children aged 5 years or under has increased modestly, but with heterogeneous trends in low and middle-income regions, while the prevalence of obesity in children aged 2-4 years has increased moderately. In 1975, obesity in children aged 5-19 years was relatively rare, but was much more common in 2016. CONCLUSIONS: It is recognised that the key drivers of this epidemic form an obesogenic environment, which includes changing food systems and reduced physical activity. Although cost-effective interventions such as WHO 'best buys' have been identified, political will and implementation have so far been limited. There is therefore a need to implement effective programmes and policies in multiple sectors to address overnutrition, undernutrition, mobility and physical activity. To be successful, the obesity epidemic must be a political priority, with these issues addressed both locally and globally. Work by governments, civil society, private corporations and other key stakeholders must be coordinated.
In 2014, the International Olympic Committee (IOC) published a consensus statement entitled “Beyond the Female Athlete Triad: Relative Energy Deficiency in Sport (RED-S)”. The syndrome of RED-S refers to: “impaired physiological functioning caused by relative energy deficiency, and includes but is not limited to impairments of metabolic rate, menstrual function, bone health, immunity, protein synthesis, and cardiovascular health.” The aetiological factor of this syndrome is low energy availability (LEA)
Soil health, along with water supply, is the most valuable resource for humans, as human life depends on the soil’s generosity. Soil degradation, therefore, poses a threat to food security, as it reduces yield, forces farmers to use more inputs, and may eventually lead to soil abandonment. Unfortunately, the importance of preserving soil health appears to be overlooked by policy makers. In this paper, I first briefly introduce the present situation concerning agricultural production, natural resources, soil degradation, land use and the challenge ahead, to show how these issues are strictly interwoven. Then, I define soil degradation and present a review of its typologies and estimates at a global level. I discuss the importance of preserving soil capital, and its relationship to human civilization and food security. Trends concerning the availability of arable agricultural land, different scenarios, and their limitations, are analyzed and discussed. The possible relation between an increase in a country’s GNP, population and future availability of arable land is also analyzed, using the World Bank’s database. I argue that because of the many sources of uncertainty in the data, and the high risks at stake, a precautionary approach should be adopted when drawing scenarios. The paper ends with a discussion on the key role of preserving soil organic matter, and the need to adopt more sustainable agricultural practices. I also argue that both our relation with nature and natural resources and our lifestyle need to be reconsidered.
Carfilzomib is a next-generation, selective proteasome inhibitor being evaluated for the treatment of relapsed and refractory multiple myeloma. In this open-label, single-arm phase 2 study (PX-171-003-A1), patients received single-agent carfilzomib 20 mg/m(2) intravenously twice weekly for 3 of 4 weeks in cycle 1, then 27 mg/m(2) for ≤ 12 cycles. The primary endpoint was overall response rate (≥ partial response). Secondary endpoints included clinical benefit response rate (≥ minimal response), duration of response, progression-free survival, overall survival, and safety. A total of 266 patients were evaluable for safety, 257 for efficacy; 95% were refractory to their last therapy; 80% were refractory or intolerant to both bortezomib and lenalidomide. Patients had median of 5 prior lines of therapy, including bortezomib, lenalidomide, and thalidomide. Overall response rate was 23.7% with median duration of response of 7.8 months. Median overall survival was 15.6 months. Adverse events (AEs) were manageable without cumulative toxicities. Common AEs were fatigue (49%), anemia (46%), nausea (45%), and thrombocytopenia (39%). Thirty-three patients (12.4%) experienced peripheral neuropathy, primarily grades 1 or 2. Thirty-three patients (12.4%) withdrew because of an AE. Durable responses and an acceptable tolerability profile in this heavily pretreated population demonstrate the potential of carfilzomib to offer meaningful clinical benefit. Concerns about problematic gaming behaviors deserve our full attention. However, we claim that it is far from clear that these problems can or should be attributed to a new disorder. The empirical basis for a Gaming Disorder proposal, such as in the new ICD-11, suffers from fundamental issues. Our main concerns are the low quality of the research base, the fact that the current operationalization leans too heavily on substance use and gambling criteria, and the lack of consensus on symptomatology and assessment of problematic gaming. The act of formalizing this disorder, even as a proposal, has negative medical, scientific, public-health, societal, and human rights fallout that should be considered. Of particular concern are moral panics around the harm of video gaming. They might result in premature application of diagnosis in the medical community and the treatment of abundant false-positive cases, especially for children and adolescents. Second, research will be locked into a confirmatory approach, rather than an exploration of the boundaries of normal versus pathological. Third, the healthy majority of gamers will be affected negatively. We expect that the premature inclusion of Gaming Disorder as a diagnosis in ICD-11 will cause significant stigma to the millions of children who play video games as a part of a normal, healthy life. At this point, suggesting formal diagnoses and categories is premature: the ICD-11 proposal for Gaming Disorder should be removed to avoid a waste of public health resources as well as to avoid causing harm to healthy video gamers around the world.
This article addresses questions of ideological polarization and the filter bubble in social media. It develops a theoretical analysis of ideological polarization on social media by considering a range of relevant factors. Over recent years, fake news and the effect of the social media filter bubble have become of increasing importance both in academic and general discourse. The article reviews the assumption that algorithmic curation and personalization systems place users in a filter bubble of content that decreases their likelihood of encountering ideologically cross-cutting news content. At the intersection of new media, politics and behavioural science, the article establishes a theoretical framework for further research and future actions by society, policymakers and industries.
BACKGROUND: Anaemia causes health and economic harms. The prevalence of anaemia in women aged 15-49 years, by pregnancy status, is indicator 2.2.3 of the UN Sustainable Development Goals, and the aim of halving the anaemia prevalence in women of reproductive age by 2030 is an extension of the 2025 global nutrition targets endorsed by the World Health Assembly (WHA). We aimed to estimate the prevalence of anaemia by severity for children aged 6-59 months, non-pregnant women aged 15-49 years, and pregnant women aged 15-49 years in 197 countries and territories and globally for the period 2000-19. METHODS: For this pooled analysis of population-representative data, we collated 489 data sources on haemoglobin distribution in children and women from 133 countries, including 4·5 million haemoglobin measurements. Our data sources comprised health examination, nutrition, and household surveys, accessed as anonymised individual records or as summary statistics such as mean haemoglobin and anaemia prevalence. We used a Bayesian hierarchical mixture model to estimate haemoglobin distributions in each population and country-year. This model allowed for coherent estimation of mean haemoglobin and prevalence of anaemia by severity. FINDINGS: Globally, in 2019, 40% (95% uncertainty interval [UI] 36-44) of children aged 6-59 months were anaemic, compared to 48% (45-51) in 2000. Globally, the prevalence of anaemia in non-pregnant women aged 15-49 years changed little between 2000 and 2019, from 31% (95% UI 28-34) to 30% (27-33), while in pregnant women aged 15-49 years it decreased from 41% (39-43) to 36% (34-39). In 2019, the prevalence of anaemia in children aged 6-59 months exceeded 70% in 11 countries and exceeded 50% in all women aged 15-49 years in ten countries. Globally in all populations and in most countries and regions, the prevalence of mild anaemia changed little, while moderate and severe anaemia declined in most populations and geographical locations, indicating a shift towards mild anaemia. INTERPRETATION: Globally, regionally, and in nearly all countries, progress on anaemia in women aged 15-49 years is insufficient to meet the WHA global nutrition target to halve anaemia prevalence by 2030, and the prevalence of anaemia in children also remains high. A better understanding of the context-specific causes of anaemia and quality implementation of effective multisectoral actions to address these causes are needed. Abstract The Beck Depression Inventory ( BDI ) is a 21‐item self‐report assessment of the intensity of depression for use in both psychiatric and nonpsychiatric clinical populations. First created by the psychiatrist Aaron T. Beck in 1961, the BDI is now in its third iteration ( BDI‐II ) and remains one of the most widely used tests of the severity of depression. The BDI‐II has been tested extensively for convergent and discriminant (construct) validities, concurrent validity, and factorial validity, and it has been translated into over 20 languages. Short forms of the BDI have been constructed for use in screening for major depressive disorders in nonpsychiatric clinical populations and for use with children and adolescents. The BDI is not a tool for diagnosing depression but rather is an assessment of the intensity or severity of depressive symptoms.

Hunters and gatherers living in tropical forests represent an important part of the total range of variation among contemporary hunting and gathering societies. Studies of tropical forest hunting and gathering peoples have contributed to our perceptions of the foraging way of life. Yet no peoples have ever been directly observed living independently of agriculture in tropical rain forest. This article tests the hypothesis that humans do not exist nor have ever existed independently of agriculture in tropical rain forest. We find no convincing ethnographic evidence and, with the possible exception of Malaysia, no archeological evidence for pure foragers in undisturbed tropical rain forests. Negative evidence cannot be conclusive, but it suggests that we need to carefully reexamine common assumptions concerning the recent history of tropical forest dwellers, the adaptability of preagricultural humans, the geographic and environmental range of hominids, and the form and consequences of selection pressures acting on humans in warm, humid environments. The overriding purpose of this article is to stimulate further ecological and archeological research in the neglected tropical forest areas of the world.
In this article, the authors reflect on the past two decades of research on organizational identity, looking to its history and to its future. They do not provide a review of the literature, nor do they promote a particular perspective on the concept. Instead, they advocate pluralism in studying organizational identity while encouraging clarity and transparency in the articulation of definitions and core theoretical suppositions. Believing there is no one best approach to the study of organizational identity, their intent is to establish a reference point that can orient future work on organizational identity. They focus on three questions they feel are critical: What is the nomological net that embeds organizational identity? Is organizational identity “real” (or simply metaphoric)? and How do we define and conceptualize organizational identity? Last, they try to anticipate organizational identity issues on the horizon to suggest future directions for theory and research.
We are currently witnessing a remarkable conjuncture between the escalation, acceleration, and diversification of migrant and refugee mobilities, on the one hand, and the mutually constitutive crises of “European” borders and “European” identity, on the other, replete with reanimated reactionary populist nationalisms and racialized nativisms, the routinization of antiterrorist securitization, and pervasive and entrenched “Islamophobia” (or more precisely, anti-Muslim racism). Despite the persistence of racial denial and the widespread refusal to frankly confront questions of “race” across Europe, the current constellation of “crises” presents precisely what can only be adequately comprehended as an unresolved racial crisis that derives fundamentally from the postcolonial condition of “Europe” as a whole, and therefore commands heightened scrutiny and rigorous investigation of the material and practical as well as discursive and symbolic productions of the co-constituted figures of “Europe” and “crisis” in light of racial formations theory.
The rapid development of machine-learning algorithms, which underpin contemporary artificial intelligence systems, has created new opportunities for the automation of work processes and management functions. While algorithmic management has been observed primarily within the platform-mediated gig economy, its transformative reach and consequences are also spreading to more standard work settings. Exploring algorithmic management as a sociotechnical concept, which reflects both technological infrastructures and organizational choices, we discuss how algorithmic management may influence existing power and social structures within organizations. We identify three key issues. First, we explore how algorithmic management shapes pre-existing power dynamics between workers and managers. Second, we discuss how algorithmic management demands new roles and competencies while also fostering oppositional attitudes toward algorithms. Third, we explain how algorithmic management impacts knowledge and information exchange within an organization, unpacking the concept of opacity on both a technical and organizational level. We conclude by situating this piece in broader discussions on the future of work, accountability, and identifying future research steps.
The World Drug Report is an annual publication produced by the United Nations Office on Drugs and Crime (UNODC) that provides information on global activity related to drug use, health, drug supply, and market analyses of opiates, cocaine, cannabis, and synthetic drugs. The report underscores drug activity as a public health and social issue that requires therapeutic intervention and treatment. The current World Drug Report estimates that over 29 million people worldwide who use drugs have a substance use disorder. In particular, women are more likely than men to engage in the nonmedical use of opioids and tranquilizers.
We consider why and how women track their menstrual cycles, examining their experiences to uncover design opportunities and extend the field's understanding of personal informatics tools. To understand menstrual cycle tracking practices, we collected and analyzed data from three sources: 2,000 reviews of popular menstrual tracking apps, a survey of 687 people, and follow-up interviews with 12 survey respondents. We find that women track their menstrual cycle for varied reasons that include remembering and predicting their period as well as informing conversations with healthcare providers. Participants described six methods of tracking their menstrual cycles, including use of technology, awareness of their premenstrual physiological states, and simply remembering. Although women find apps and calendars helpful, these methods are ineffective when predictions of future menstrual cycles are inaccurate. Designs can create feelings of exclusion for gender and sexual minorities. Existing apps also generally fail to consider life stages that women experience, including young adulthood, pregnancy, and menopause. Our findings encourage expanding the field's conceptions of personal informatics.
