Intersecting Vulnerabilities: The Impacts of COVID-19 on the Psycho-emotional Lives of Young People in Low- and Middle-Income Countries

Across diverse contexts, emerging evidence suggests that the COVID-19 pandemic is increasing levels of anxiety and stress. In calling for greater attention to people's psychosocial and emotional well-being, global actors have paid insufficient attention to the realities of the pandemic in low- and middle-income countries, where millions of people are already exposed to intersecting vulnerabilities. Chronic poverty, protracted violence, conflict and displacement, coupled with weak health, education and protection systems, provide the backdrop of many adolescents' lives. Drawing on qualitative in-country telephone interviews with over 500 adolescents in Ethiopia, Côte d'Ivoire and Lebanon, this article unpacks the age and gendered dimensions of COVID-19 and its response. We conclude by discussing the implications for COVID-19 recovery efforts, arguing that embedding adolescent-centred, inclusive approaches in education, community-based health and social protection responses, has the potential to mitigate the psycho-emotional toll of the pandemic on young people and promote resilience.

Variability of the mixed layer heat budget in the eastern equatorial Atlantic during 2005–2007 as inferred using Argo floats

[1] We examine the variability of the sea surface temperatures in the eastern equatorial Atlantic during 2005–2007 by using Argo profiling floats, Prediction and Research Moored Array in the Atlantic buoys and satellite, in situ, and atmospheric data sets. The eastern equatorial Atlantic, characterized by shallow mixed layers all yearlong, is divided into nine boxes of nearly equal surface area, with respect to the dynamics and thermodynamics in this region. Monthly mixed layer heat budgets are computed in each box from 10 day Argo profiles. In all the boxes, the net surface heat flux is one of the main causes of the seasonal evolution of sea surface temperatures for the 3 studied years. The amount of short-wave radiation penetrating through the base of the mixed layer and horizontal heat advection may locally contribute to the temperature variability, while entrainment has a weaker contribution. To balance the heat budget, a residual term exists which includes all processes that cannot be calculated with observations as well as the possible errors in the other terms. This residual is more intense in the cold tongue and the northern region and exhibits a clear seasonal cycle, with minimum (negative) values in boreal summer and maximum values in winter. This residual compares well with available observations of vertical turbulent mixing collected during Etude de la Circulation Océanique et des Échanges Océan-Atmosphère dans le Golfe de Guinee campaigns (2005–2007) in the eastern equatorial Atlantic. When assuming that the residual is mostly associated with vertical turbulent mixing, it can be conjectured that turbulent mixing is a significant cooling source in the cold tongue and north of the equator.

How to recruit and retain health workers in underserved areas: the Senegalese experience

PROBLEM: Significant regional disparities in human resources for health deployment in Senegal weaken the country's health system and compromise population health. APPROACH: In recent years, the Ministry of Health adopted measures to improve the posting, recruitment and retention of health workers in rural and remote areas. One was the introduction of a special contracting system to recruit health workers. LOCAL SETTING: Health workers in Senegal are concentrated in specific urban centres, particularly Dakar. Whereas the Dakar region has 0.2 physicians per 1000 population, the Fatick, Kaolack, Kolda and Matam regions have fewer than 0.04. The density of midwives and, to a lesser extent, of nurses also varies considerably among different regions in Senegal. RELEVANT CHANGES: Between 2006 and 2008, the introduction of the special contracting system contributed to the successful recruitment of health workers in remote and rural regions and the reopening of health outposts. LESSONS LEARNT: The introduction of a special contracting system for health workers was a successful approach to reopening health posts in regions with low health workforce density in Senegal. However, the long-term sustainability of such an approach, particularly in fiscal terms, must be considered, as a single policy intervention may not be enough to address the diverse and complex challenges in human resources for health facing different regions of Senegal.

Standards in wildlife forensic science, with a focus on non‐human DNA analysis

For genetic data to be used in forensic casework, it has to be produced within a controlled environment that follows strict quality standards. However, recent reviews have suggested that wildlife forensic laboratories are behind in the development and adherence to appropriate standards for casework. This paper will address these concerns by documenting the standards that have been produced, highlighting the systems of assessment and competency testing available, and reviewing the status of validated reference genetic databases. Networks of dedicated wildlife forensic scientists across the globe, represented in part by the author list for this paper, illustrate the strides taken to build capacity in this field, and an ongoing commitment to present quality wildlife forensic evidence in court.

Monovalent type 2 OPV (mOPV2) management in the field: Interventions and lessons learned

The polio endgame strategy calls for ending the use of and removal of all Sabin vaccines globally given the risks of generation and spread of cVDPVs. With the successful eradication of wild poliovirus type 2 in 2015, the process of removing type 2 Sabin vaccines began with the switch from tOPV to bOPV across national vaccination programs. Following the tOPV to bOPV switch in April/May 2016, monovalent type 2 OPV (mOPV2) has been put into use in response to detected cVDPV2 polioviruses outbreaks. Between 31 May 2016 and 30 Jun 2020, 453 million doses of mOPV2 were provided to 21 countries to conduct 235 campaigns to respond to cVDPV2 outbreaks and high-risk events. However, the use of this vaccine paradoxically reintroduces live attenuated type 2 poliovirus into the populations and the environment, therefore, poses a risk for the emergence of new VDPV2s. Thus, it is critical to carefully and appropriately manage all in-country mOPV2 stocks utilized in outbreak response to minimize this risk. In this article, we examine the performance of mOPV2 vaccine management utilized for various outbreak responses after the switch.We present the major challenges faced and the lessons learned, to improve technical guidance and future response activities. Performance varied significantly across countries in terms of each of the activity areas evaluated. There were major gaps, especially in terms of vaccine accountability, and in many instances large numbers of vials went unaccounted presenting additional risk for further VDPV2 emergences. We have shown that especially at the beginning of implementation, insufficient attention has been given to mOPV2 vaccine management. Enhanced focus on mOPV2 vaccine management in line with the lessons learned presented in this paper should be a priority for public health programs and countries to consider and adapt in future VDPV2 responses as well as potential future activities associated with eventual complete withdrawal & cessation of OPV.These experiences can also be extended to other vaccines for which strict stock management and containment measures are required.

Redescription of Liza bandialensis (Teleostei: Mugilidae) with an identification key to mullet species of Eastern Central Atlantic

Liza bandialensis Diouf 1991 is redescribed because previous descriptions have not been in well-distributed publications and have lacked sufficient detail or reference to voucher specimens. The description provided here is based on specimens from the Sine Saloum estuary, Senegal (West Africa), from where the species was originally described. The distinctness of the species is confirmed both by meristic and molecular criteria. L. bandialensis presents a unique combination of characters with a low number of scales in the longitudinal series (32-33), 10.5-12 transverse scale rows, and distinctly yellowish dorsal, anal, and caudal fins. The currently known distribution of L. bandialensis includes coastal waters of Senegal, Gambia and Guinea Bissau. Finally, we provide a morphological identification key for the sixteen species of Mugilidae species occurring along the eastern central Atlantic coast of Africa.

Sequelae following an epidemic of meningococcal meningitis in Niger

INTRODUCTION MSF supported Niger’s Ministry of Health (MoH) in responding to a serogroup C meningococcal meningitis epidemic in Magaria and Dungass Districts in 2022. WHO’s global roadmap for defeating meningitis by 2030 emphasises appropriate care for meningitis sequelae, but this is not yet part of standard epidemic response. Meningitis sequelae in the African meningitis belt are poorly described, hampering access to rehabilitation services. To better orient future care for sequelae, we performed a follow-up survey of survivors 9 months after the 2022 epidemic. METHODS WHO case definitions were used during the epidemic. Patient-level line-lists detailing village of origin were obtained from authorities, and results of polymerase chain reaction testing on cerebrospinal fluid were integrated. Guided by village leaders, study nurses attempted to visit cases at home to assess for sequelae. Nurses administered questionnaires asking about history including seizures and subjective vision and hearing loss; and carried out physical examinations assessing anosmia, weakness, and paralysis. Data were collected tablets using REDCap software. Prevalence of sequelae among survivors was calculated. ETHICS This study was approved by the MSF Ethics Review Board and by the National Ethics Committee for Health Research of Niger. RESULTS 1001 suspected cases and 50 deaths (case fatality rate, CFR, 5.0%) originating in 230 villages were recorded on the line-lists. 469 cases (47%) had lumbar puncture, and 220 (47%) had a causative agent identified, including 192 cases of Neisseria meningitidis serogroup C (NmC) and 22 Streptococcus pneumoniae. After excluding 82 cases living in villages difficult to access, we attempted to trace 919 cases, of whom 570 (62%) were found and consented to interview. Among these cases, 49 had died (CFR 8.6%). Among the cases visited, 151 had confirmed NmC and 10 S. pneumoniae. Among the 521 surviving cases evaluated, 62 (12%) had sequelae; the most common were hearing loss (29), paralysis (16), epilepsy (9), and developmental regression (6). Among the 138 surviving confirmed cases of NmC, 25 (18%) had one or more sequelae. CONCLUSION We documented a higher CFR than reported during the epidemic, and a high burden of sequelae among survivors, particularly among those with confirmed NmC infection. To our knowledge, this is the second time that meningitis sequelae have been documented in Niger; these findings help identify priorities for integrating meningitis after-care into epidemic responses. MSF and the MoH should work to ensure linkages to long-term care and support for meningitis survivors and their caregivers. We were unable to find all cases, so the true prevalence of sequelae among survivors may differ. This follow-up survey used simple methods adapted for in-home evaluation, and formal audiometry may have led to detection of more subtle hearing loss. CONFLICTS OF INTEREST None declared

Complexity in trophic structure and stability in a modelled West African estuary

Un modèle Ecopath de l'estuaire du Sine-Saloum (Sénégal) comportant au total 37 groupes fonctionnels a été appliqué dans la présente étude en vue de déterminer les faits essentiels de ce système aquatique. La grande biodiversité spécifique des poissons de l'écosystème peut y jouer un rôle en assurant son intégrité, spécialement en présence de facteurs de stress, bien qu'il ait été observé que ce n'est pas entièrement le cas. Les résultats montrent qu'un autre facteur de mitigation est le niveau des forces d'interaction entre les groupes fonctionnels. La stabilité est accrue par des relations plus faibles et des pyramides plus courtes et plus larges. Bien que différentes questions se posent concernant les facteurs conduisant à la stabilité d'un système, les méthodes et les façons de les approcher devraient envisager d'en situer les résultats dans le contexte d'une théorie écologique plus large.
